By submitting this form you agree to the following - Dog(s) will be kept on a lead at all times & fouling cleaned immediately. You are bound by the Czechoslovakian Vlciak Club UK Constitution & must abide by the Event Regulations. No dog which displays or has been knowingly exposed to any infectious or contagious disease during 21 days prior to event date, or suffering from a visible condition which adversely affects health or welfare is permitted. You agree without reservation that any veterinary surgeon operation on any dogs which alters natural conformation must be disclosed prior to the event. Dogs in breach of Czechoslovakian Vlciak Club UK Constitution are liable to disqualification. Children under the age of 12 must be accompanied at all times by a Parent or Guardian.
